What is a Dual Eligible Special Needs Plan (D-SNP)?
A Dual Eligible Special Needs Plan (D-SNP) is a specific Medicare Advantage (Part C) plan designed for individuals who are dually eligible for both Medicare and Medicaid. D-SNP provides a convenient way to manage healthcare coverage under one policy with extra benefits and more services than the standard coverage in Medicaid and Original Medicare (Parts A & B).

Receive assistance with specialized care needs, like completing a Health Risk Assessment, managing health conditions at home, medication management, support adjusting to life at home after a hospital stay, guidance on over-the-counter (OTC) products, and more.

Community Associate: Helps make appointments, referrals, schedule transportation rides, and order prescriptions, supplies and equipment.

Social Worker: Helps connect to community resources, like housing, food assistance, utility assistance, Medicaid long-term care applications and more.

Medicaid Eligibility Specialist: Helps access Medicaid benefits, verify eligibility qualifications and annual renewal, and apply for federal and state assistance programs, like Extra Help and food stamps. Receive assistance in verifying active eligibility for Medicaid benefits each month as required by the program.

Enrolling in D-SNP coverage
If you’re eligible for Medicare and Medicaid you may have the flexibility to join, switch or drop your Medicare Advantage (Part C) or Prescription Drug (Part D) coverage during:
Annual Enrollment Period
- October 15–December 7
- changes take effect on January 1
Special Enrollment Period
- One time during each period: January–March, April–June, July–September
- Changes take effect on the first day of the following month. You’ll have to wait for the next period to make another change.
